Documentation For Volunteers

• Patient Visit Report [2]. Please submit this form within 24 hours of your patient visit.
   
• Cancelled Patient Visit Report [3]. Submit this form if your patient visit is cancelled.
   
• Documentation Guidelines [4]. This is a quick reference guide for patient care visit documentation compliance and standards.
   
• Assignment Data Sheet [5]. This form is for patient care volunteers to record information when a new patient assignment is received.
   
• Volunteer Time Sheet [6]. Use this form to report your volunteer time for activities that do not involve patient care.

• Volunteer Handbook [7]. The San Diego Hospice and The Institute for Palliative Medicine Volunteer Handbook is a reference guide for all volunteers. Go green! Rather than printing your manual, you may check this online version at any time. Updates will be posted promptly if there are any changes.
   
• TB Skin Test Information. [8] An Annual TB Skin test is required for all volunteers with patient contact.  Find out where you can get a free TB skin test so that you can join our volunteer workforce, or maintain your volunteer status.
   
• Annual TB Symptom Survey/Questionnaire Form. [9] Complete this questionnaire before your TB test at San Diego Hospice and The Institute for Palliative Medicine.
